Clinic Policies
In choosing to receive care at Ravenswood Rheumatology, you are agreeing to our policies, outlined below. These have been created with a focus on safety, accessibility and building a therapeutic relationship between our patients and our team.
Treatment of others

We understand that navigating the medical system is stressful and we will always treat you with respect on this journey. We ask and expect the same in return for our staff as well as our providers. Verbal or physical abuse, threats or insults to any member of our team will not be tolerated under any circumstances and we will not be able to continue seeing you as a patient if this occurs.
Appointments

Due to a large volume of patients and to uphold our commitment to keeping clinic accessible in the face of numerous demands on Dr Jain's time, your follow-up appointment will be scheduled with one of our highly experienced PAs. As always, in keeping with our team-based approach, care plans are reviewed with Dr Jain.

If you are an infusion patient, please make sure to also make clinic appointments with the front desk at time intervals instructed by your provider.

We require 24-hour notice of cancellation for scheduled appointments as that time can be used for another patient who is waiting to be seen. Missed appointments, or appointments that are canceled with less than 24-hour notice will be charged $30. This applies to both video and in-person visits.

If you will be more than 15 minutes late to your appointment please call our office to see if we are still able to accommodate you or if we will need to reschedule you. If you do not notify us that you are running late, your appointment will need to be rescheduled and this will be considered a no-show.

If you have an HMO insurance policy, you are responsible for providing updated referrals from your primary care provider as required by your individual policy.
Telehealth

In order to qualify for a telehealth visit, you must have had an in-person visit within the last 6 months.

We prioritize in-person visits as regulations around telehealth have tightened
Medication Refills

For liability and safety reasons we absolutely cannot refill meds if you have not been seen in-person and had updated lab work within the last year.

Certain medications require a visit and/or labs every 3-6 months to be safely refilled.
FORMS

For us to accurately fill out any paperwork/forms, you need to have been seen in our clinic within the past 6 months

Forms will be filled out within 7 days of receipt and do incur a processing fee.
